Care plans

Assign home programmes with exercises, diet instructions, speech drills, oral hygiene, medication routines, general instructions and reusable templates.

Who can do this: careplan.read views plans and templates; careplan.manage assigns, edits, completes, deletes and manages templates. Where: care plans (/app/care-plans?patientId=:patientId&patientName=:name), templates (/app/care-plans/templates), portal (/patient/care-plan). Legacy /app/home-exercises and /patient/exercises use the same feature.

Before you start​

  • Open the patient first so patientId is present.
  • Prepare instructions and approved HTTPS media links.
  • Use templates for common programmes.

Open a patient's care plans​

  1. Open the patient and choose the care-plan action. Result: /app/care-plans?patientId=... opens.
  2. If No patient selected appears, click Go to Patients. Result: You return to the patient list.
  3. Review existing cards. Result: Current and completed plans are visible.

Assign a care plan​

  1. Click Assign new. Result: New: Care plan opens.
  2. Enter Title, Goals (one per line), Start date and optional End date. Result: Plan basics are ready.
  3. Add at least one activity with Type, Name, instructions, frequency, duration, sets, repetitions or media link. Result: The patient has actionable tasks.
  4. Add Notes for patient / family. Result: Precautions and motivation are included.
  5. Click Assign. Result: Assigned appears and the plan card is listed.

Start from a template​

  1. Click Assign new. Result: Start from a template appears when templates exist.
  2. Choose a template and click Add activities. Result: Activities and goals are copied into the draft.
  3. Edit for the patient. Result: The plan is personalised.
  4. Click Assign. Result: The plan is saved.

Edit, complete or delete​

  1. Click Edit on a card. Result: Edit: Care plan opens.
  2. Change fields and click Save changes. Result: Updated appears.
  3. Click Mark complete. Result: Marked as completed appears.
  4. Click Delete and confirm Delete this plan? only for mistakes. Result: The plan is deleted.

Manage templates​

  1. Open Care plan templates. Result: Built-in and tenant templates appear.
  2. Filter by Specialty or All specialties. Result: The list narrows.
  3. Click New template. Result: Template form opens.
  4. Enter name, specialty, description, goals and activities. Result: A reusable activity set is ready.
  5. Click Save template. Result: Template saved appears.
  6. Click Save a copy on a built-in template to adapt it. Result: A tenant copy opens.

Patient portal​

  1. Ask the patient to open /patient/care-plan. Result: My care plan appears.
  2. The patient reviews assigned plans, activities, notes and tips. Result: The family can follow the home programme.
  3. The patient opens media links. Result: Approved links are available; unapproved links are hidden.
