R1 — मल्टी-स्पेशलिटी क्लिनिक (OPD)

R1 में Achal Healthcare therapy-focused app से बदलकर भारत के clinics के लिए multi-specialty OPD platform बनता है। इसमें सामान्य patient identity, queues, visits, prescriptions, clinical documents, GST billing, cash controls, letterheads, offline drafts, AI assist, onboarding, imports, exports और दो-भाषी documentation शामिल हैं।

प्रमुख बातें​

  • Facility presets, specialty packs, module registry, RBAC और capability-driven navigation के साथ multi-specialty setup।
  • UHID-based patient identity, जिसमें duplicate WARN या BLOCK policy, merge और undo, restricted records और workspace switching है।
  • Vitals, ICD-10 problems, allergies, Rx, files, consents, vaccination और telemedicine के लिए encounter workspace।
  • पुराने report generators की जगह clinical documents, जिनमें bilingual templates, e-signature, amendments, addenda, PDF, QR verification और share links हैं।

  • Versioned facility presets, specialty catalog, module registry, terminology layer और /me/capabilities payload menus, labels, modules और role-based access चलाते हैं।
  • Permission catalog patients, appointments, queue, encounters, clinical documents, templates, Rx, billing, finance, settings, branding, documents vault, consent, immunization, teleconsultation और AI assist को cover करता है।
  • पुराने tenants के लिए THERAPIST, Practitioner का alias बना रहता है; नई wording specialty के हिसाब से चलती है।
  • Navigation, queue, clinical documents, editor, print, encounter, revenue, care plans, templates, offline और AI assist के लिए English और Hindi i18n resources registered हैं।
  • New modules में बहुत बड़ी lists बिना सीमा के न पढ़ी जाएँ, इसलिए cursor pagination और declared query shapes जोड़े गए।

मरीज़ की पहचान और वर्कस्पेस​

  • Tenant-specific prefix और sequential counters के साथ UHID allocation और backfill।
  • Duplicate policy: existing tenants BLOCK रखते हैं; new onboarding tenants WARN इस्तेमाल करते हैं और reason देकर override कर सकते हैं।
  • Possible duplicate panel phone, UHID और birth year mask करता है; restricted या other-branch matches नहीं दिखाए जाते।
  • Patient merge, resume और undo linked records move करते हैं, जिनमें appointments, bills, queue tokens, clinical documents, encounters, vitals, allergies, prescriptions, packages और privacy requests शामिल हैं।

विज़िट, पर्चे और क्लिनिकल वर्कस्पेस​

  • Visit workspace /app/patients/:patientId/encounters/:encounterId पर है, जिसमें Vitals, Problems, Allergies, Prescription, Documents, Files और Vaccination tabs हैं।
  • Vitals validation असंभव values रोकता है और BMI calculate करता है; problem search ICD-10 subset इस्तेमाल करता है; allergy banners prescription warnings में जाते हैं।
  • Rx pad generic या brand search, favourites और sets, bilingual instructions, warnings, NMC telemedicine rules, registration-number checks, print और WhatsApp portal links support करता है।

  • E-consent templates, guardian consent rules, IAP vaccination card, due और overdue status, reminders और teleconsultation start उपलब्ध हैं।

क्लिनिकल दस्तावेज़, टेम्पलेट, पैक और विजेट​

  • @achal/clinical-core restricted JSON Schema, UI schema, computed functions, conditions, macros, smart phrases, copy-forward और canonical hashes validate करता है।
  • Clinical templates library packs, tenant clones, draft, publish, archive, immutable versions, lint checks, normal macros और smart phrases support करते हैं।
  • Clinical documents draft autosave, sign, co-sign, amend, addendum, void, event history, access log, document numbers और PDF outbox support करते हैं।
  • Specialty packs में General OPD, therapy, psychiatry and psychology, consents, dental, ENT, gynaecology, nutrition, ophthalmology, pediatrics और dermatology templates हैं।
  • Specialty widgets में tooth chart, body map, audiogram, growth chart, milestone grid, ROM/MMT grid, scale और sensory quadrant हैं।

रेंडरिंग, PDF, लेटरहेड और साझा करना​

  • Canonical server HTML signed documents को escaped content, repeated letterhead, page numbers, status watermark और ज़रूरत होने पर Noto Sans Devanagari के साथ render करता है।
  • /app/settings/brand-profiles पर brand profiles default या branch letterheads, logo, signature, seal, paper size, margins, themes, pre-printed paper और verification QR support करते हैं।
  • PDF rendering outbox और renderer service इस्तेमाल करता है; users तुरंत print कर सकते हैं और PDF ready होने पर download कर सकते हैं।
  • /verify/:token पर QR verification document number, issuer, signed date और status दिखाता है; patient या clinical details नहीं दिखाता।
  • Share links expire होते हैं, patient के mobile पर one-time code माँग सकते हैं, WhatsApp पर भेजे जा सकते हैं और revoke किए जा सकते हैं।

बिलिंग, GST और वित्त​

  • GST profile, service tax category और buyer GSTIN GST-exempt healthcare, taxable cosmetic services और taxable goods support करते हैं।
  • Bills GST server-side compute करते हैं और Tax Invoice, Bill of Supply या Invoice-cum-Bill of Supply के रूप में print होते हैं।
  • Credit notes full, partial और selected-line credit support करते हैं, proportional GST reversal और refund record के साथ।
  • Daily cash closing में cashiers counted amounts submit करते हैं, managers approve या reopen करते हैं, और late adjustments visible रहते हैं।
  • UPI reconciliation हर payment पर UTR एक ही बार record करता है; practitioner payout statements GST और credit notes छोड़कर billed या collected share compute करते हैं।

केयर प्लान, ट्रीटमेंट पैकेज और पोर्टल दृश्य​

  • Home exercise programmes को care plans में generalise किया गया है और legacy adapter रखा गया है।
  • Treatment packages therapy sessions से आगे typed packages support करते हैं और session-package compatibility बनाए रखते हैं।
  • Patient portal selected patient profile के लिए prescriptions, vaccination card और care plan content दिखाता है।

ऑफ़लाइन PWA और AI assist​

  • Offline page और update prompt के साथ installable PWA shell; service worker API और PHI cache नहीं करता।
  • Encrypted IndexedDB drafts user session और tenant से bound हैं; logout या tenant switch local records wipe करता है।
  • Offline support clinical document draft PATCH queue, walk-in token queue और patient-registration draft restore cover करता है।
  • AI assist tenant DPA acknowledgement, module gating, metering और sensitive-record blocks के साथ opt-in है। Configured होने पर dictation, clinical draft proposals, summaries और ICD suggestions मिलते हैं।

  • Signup facility type, specialties और preferred language लेता है, फिर clinical configuration, WARN duplicate policy, UHID prefix और onboarding state provision करता है।
  • /app/onboarding में skippable steps हैं: clinic, departments, practitioners, branding, services, templates, gst, import और goLive, साथ में demo-data toggle और go-live checklist।
  • /app/data/import केवल CSV UTF-8 support करता है, 3 MB और 20,000 rows तक, mapping, dry-run validation, resumable commit और rollback के साथ। XLSX import supported नहीं है; file को CSV UTF-8 के रूप में save करें।

  • डॉक्स साइट अब अंग्रेज़ी और हिंदी (/hi/) में है: शुरुआत करें, भूमिका गाइड, क्विक-स्टार्ट कार्ड, FAQ और रिलीज़ नोट्स का हिंदी अनुवाद है; मॉड्यूल, एडमिन और टेम्पलेट गाइड अंग्रेज़ी में दिखती हैं। छह प्रिंट करने लायक क्विक-स्टार्ट कार्ड और दस द्विभाषी वीडियो-ट्यूटोरियल स्क्रिप्ट भी शामिल हैं।
  • In-app help stable doc ids पर deep link करता है। R1 में ? से खुलने वाला help panel, Shift+? shortcut, queue, encounter, clinical documents और billing के per-module guided tours, और release changes के लिए What’s new dialog जोड़ा गया है।

माइग्रेशन नोट्स​

  • Legacy clinical reports और OT reports stream w4-detherapy-legacy की migration script से clinical documents में migrate होते हैं। इसे platform team चलाती है; clinics को कोई action नहीं लेना है।
  • THERAPIST, Practitioner के alias के रूप में चलता रहेगा। Role wording अब specialty terminology से आती है।
  • Existing tenants BLOCK duplicate policy और अपनी default clinical configuration रखते हैं, जब तक owner उसे बदल न दे।
  • Legacy plan codes grandfathered हैं: BASIC, Clinic से map होता है; PRO और ENTERPRISE, Polyclinic से map होते हैं, और जहाँ पहले agree किया गया था वहाँ ENTERPRISE limits unlimited रहती हैं।
  • HEP data care plans legacy adapter से खुलता है।
  • Session packages treatment packages के through backward compatible रहते हैं।
  • Existing signed documents, bills, appointments, packages और audit logs अपने original identifiers रखते हैं।

बदलाव जिनसे पुराने तरीके टूट सकते हैं​

  • Legacy report stack की जगह Clinical documents आ गए हैं। पुराने report generator pages हट गए हैं: /app/reports, /app/ot-report, /app/initial-ot-evaluation, /app/ot-progress-report, /app/pt-initial-evaluation, /app/developmental-milestone, /app/discharge-summary, /app/clinical-report/print, /app/ot-report/print।
  • API /patients/clinical-reports* अब 410 Gone return करता है। Integrators को /clinical/documents और related clinical document APIs इस्तेमाल करनी होंगी।
  • localStorage report print handoff हट गया है। Printing अब server-rendered clinical document HTML और print या PDF page से होती है।
  • Final clinical documents और finalized prescriptions immutable हैं। Corrections के लिए amendment, addendum, void, credit note या नया prescription इस्तेमाल करें।
  • CSV import केवल CSV UTF-8 accept करता है; XLSX files upload करने से पहले CSV UTF-8 में save करनी होंगी।
  • Public display, self check-in, verify और share links tokenised और revocable हैं। पुराने unauthenticated assumptions integrations में इस्तेमाल नहीं करने चाहिए।

ज्ञात सीमाएँ और जल्द आने वाली सुविधाएँ​

  • Lab और radiology workflows R2 के लिए planned हैं।
  • Network और cross-tenant exchange workflows R3 के लिए planned हैं।
  • Pharmacy और hospital workflows R4 के लिए planned हैं।
  • R1 में XLSX import supported नहीं है। Excel या Google Sheets से Save as CSV UTF-8 इस्तेमाल करें।
  • Vaccine reminder scheduler, tenant-editable vaccination schedules, tenant-editable telemedicine drug lists और built-in video infrastructure deferred हैं।
  • PDF generation में PDF is being prepared… दिख सकता है जब तक render job खत्म न हो; print तुरंत उपलब्ध रहता है।

क्लिनिक मालिकों के लिए अपग्रेड चेकलिस्ट​

  1. Clinic profile, branches और GST settings review करें।
  2. Clinical configuration में specialties, enabled modules और terminology confirm करें।
  3. Staff training के बाद तय करें कि BLOCK duplicate policy रखनी है या WARN पर जाना है।
  4. Prescriptions और signed documents इस्तेमाल करने से पहले practitioner registration numbers जोड़ें।
  5. Letterhead brand profiles बनाएँ या review करें और एक demo clinical document print करें।
  6. Reception को queue, self check-in, appointment check-in और duplicate warnings पर train करें।
  7. Clinicians को visit workspace, prescription warnings और clinical document signing पर train करें।
  8. Go-live से पहले एक demo bill, payment, credit note और cash closing run करें।
  9. Data import करते समय files CSV UTF-8 में save करें, dry-run results validate करें और rollback window clear रखें।
